I would love it if you could assist me in re-enabling my disabled form. I fixed the fields and would greatly appreciate a quick response as I just send out a bunch of emails. Thank you!

I checked your form, as well as your account and both appear to be active. You form does not seem to be disabled any more. If you have any further trouble with your form, let us know and we will be happy to have another look.
