I do not remember ever setting up a user name and password which your system says I have. I request to reset my password and it says an email is sent but I never get one. Please help me resolve this, I cannot set up a new account since my email is already in your system and I can't reset the password since I never receive a email in my inbox or junk.

This form (http://www.jotform.com/52655030257148) is associated to a guest account (guest_52584851288063). It means that you did not registered an account in JotForm. So you can't reset any password because you don't have an account with us.
Please sign up or registering an account. After that, we can go ahead and transfer this form to the registered account.
I also removed your email address from the bounce list.
