I have had several clients in the past couple weeks that have said they submitted an appointment request via JotForm and we have not got an email notification to tell us this. At first we got an email every time to alert us. Is there a reason this is happening? How do I remedy this problem??

I took a look at the form and it seems to have an issue in its setup:
The sender name should be set to some other value, like Full Name.
Based on our logs it seems that your emails are already being sent since today, but correcting this issue as well will make sure that you avoid other possible issues like having the emails ending up on bounce list, etc.
