It is actually showing as disabled. Please help!
It looks like you have set up Form Limits on your form, so it was automatically disabled when its limits were met. Please see the attached image below:
You can enable your form again by following our guide on How to Enable/Disable a Form.
I would also advise you to remove the form limits if you did not intend to set them up, as form limits may disable your form again.
