how do I insert or create a page in my jotform to be inserted into my website to be a printable form?

I have a form named Health Certificate that I would like for my clients to be able to print out. I want to have them click on the button that says print health certificate and it print for them.  How is this done?

It is easy to add print button on the forms, follow this guide: http://www.jotform.com/help/24-Print-Button-on-Forms

So when they fill the form and click on the print button, it will show the printable version of the form with their data in it.
