Form disappeared from website

The contact us form disappeared from the website and I am not even able to see it on my 'my forms' page on jot form website either. This morning I received an email saying I needed to upgrade because there are more than 100 forms entered a month. I paid the fee and not sure why the form disappeared?

Your forms should be working fine by now. It was due to a temporary server downtime but it has been fixed right away. I can see the form on your page now.
