Is it possible to integrate JotForm with PMPRE4 and create a form that can replace the "t_profile_email.htm" template and allow one to send an email to %%login%%?

PM Premium already have this feature. Just add %%TEMPLATE:profile_email%% to any page and it will create a email sending form that uses PM Premium. So, no need to use JotForm for it.

What I actually need to do is replace the PMP4 profile_email template, with a JotForm and have the information
that is entered into the JotForm, sent to the member. Something like:

Hope it's possible to do.

No, it wouldn't be that easy.
You can create a form on JotForm and add an auto-responder to it. Auto-responder's recipient address would be a hidden field. (You can add a hidden field under "Power Tools" on JotForm)
Then you can populate this hidden field when you add the jotform form to the PM Premium template.
