    Hi,

    To disable day field from the date, you need to use the "Configurable List" widget instead of "DateTime" field from the "Quick Tools".

    To do that,

    1) Click to "Widgets, then write "Configurable List" on the Widget's search box. The "Configurable List" widget will appear, click on it to add it to your form.

    2) Once you added it on your form, there will be settings pop up window appear. On the "General Settings"  tab, erase the unnecessary things. In this case, just leave the "Calendar : date : m/d/y : 2000-2015" row.

    3) On the "Custom CSS" tab, you need to add following CSS code to disable day field from configurable list's date. Finally, click to "Finish".

    .d, .calendarIcon {

    display: none;

    }

    The final look of the widget is just like the image below:

    Also, you find the demo from the following link:

    If you stuck somewhere in the process or need more information, please let us know.

    Have a nice day.

    @bestsingletravel

    I have inspected your form: http://www.jotform.com/form/11202940174 and I noticed that you have fields that collect credit card information and that is against JotForm Terms of use.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    You need to remove those fields from your form. If you not remove them, we will need to do it.

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

    I have inspected your other forms and most of them has fields that collect credit card info.

    You need to remove fields that are used for collecting credit card information from all of your forms.

    If you do not remove those field in next 24 hours we will need to do it.

    Termination. You agree that Company may terminate your JotForm Service membership or suspend your access to all or part of the JotForm Service, without notice, if Company determines, in its sole and absolute discretion, that you have violated these Terms. Further, you agree that Company shall not be liable to you or any third party for removing your Submissions or suspending or terminating your access to the JotForm Service. You may discontinue your participation in and access to the JotForm Service at any time.

    Considering that you did not delete fields that collect credit card info I have logged in to your account and deleted mentioned fields from all of your forms.

    If we notice that you have added the fields back, we will need to delete the forms completely, together with all submissions.

    Once again if you want to have payment fields inside of your forms you have to use one of JotForm payment tools.

    Thanks for the big warning - I was out of the office yesterday and it takes me more then 24 hours to change all the links on my website.

    I have been a customer for years and this is how you to treat a loyal customer. You could have at least given us the weekend.

    Hi Sheryl.

    I do understand that you were out of the office and that you might not have been able to reply back to us in regards to this.

    Please do note that if it was me, or some other of my colleagues we would have removed your forms completely since our terms of use indicate that we should do just that as soon as we find any form that is breaking the same - no matter how long someone is with us or if they are on free or paid plan.

    This would mean that all of your data captured on the form would be completely removed. My colleague above had actually taken a longer way of doing this in order to provide you with more support so that you do not need to go into each of over 40 forms by yourself and make these same changes manually.

    This means that she has probably saved you an hour or more of work by removing the fields for you (which you would have have to do by yourself).

    In the same time, I can see that your forms would have already been removed yesterday since our system had detected that you are collecting these details as it was monitoring what was going on so if the 24 hour delay was not already been given, you are likely to have been sent an email of your forms being removed and the reason of the same with the link to the terms of use that show why that has happened.

    This means that you would have needed to recreate the entire forms - which I have seen already happen several times when many forms were breaking the terms of use in a similar way.

    Having all that said, I would like to mention again, that instead of spending weekend to remove the fields, my colleague has done this for you, so you do not need to make any changes to your forms - just to add the payment processing tools as mentioned above.

    If you happen to have any questions in regards to the payment tools integrations, please do let us know by opening a new thread and we would be happy to assist with the same.
