4. Our Riders can create their company profile on their sign up tab. On that page a list of their driver's with car and plate number, contact info, picture trips, amounts, this will be picture file that will be uploaded manually from our Admin site.

We can change the location of the tabs bus basically same function.
I would first like to suggest to create a free plan before creating any forms. This will allow you to have the form in that account and be able to return to it at all times.

Now, in regards to your question. I, as my colleagues are always happy to assist, but I am not quite sure what you mean here.
If you are looking to accept the values such as:
1. Full name
2. license plate number
4. contact phone
This would be easy to do by utilizing the Configurable list so all of these fields can be repeated per rider.
Now, based on the part that follows the list, it seems that you are actually looking to upload an image that will hold all of these details instead.
If that is true, then you can simply add the upload field to your form and upload the image to it.
As mentioned, once we know more details, we would be able to properly assist you with the same.
