    Want to send forms via email and ideally have the send from and subject line be controllable

    We send PDF forms as attachments and find our return rate is around 25%. My only assumption is that the effort of opening attachments, saving the updated PDF and emailing the completed form back is to arduous. I'd like to be able to email forms. Here are my requirements in the rough. Can you assist?

    1: form embedded in the email so responders can fill the form out without opening an attachment or going to another site.

    2: at a minimum, clicking on reply would go to a specific address from a known address with a definable subject line based on what was sent to the user.

    1: form embedded in the email so responders can fill the form out without opening an attachment or going to another site.

    Unfortunately, it is not recommended to have form fields inside an email message. Email provider or programs usually blocked it. You can send an HTML email but like I said, it is possible that the email will not be sent or will encounter a problem.

    You can embed the form using the normal embed or the iframe method on the HTML email you will create.

    I'm sorry but what "reply" button are you referring here? All the settings regarding the notifications or autoresponder can be change in the email settings.
