My form was functioning for about a month. The form loads, but when information is filled out and the submit button is pressed, an error page returns the message "Form Disabled: This form has been disabled". Then, I found that I could "enable" my form at http://www.jotform.com/myforms. I enabled the form, but the form now results in the error message "This form is currently unavailable!".
How do I get my form working again?

Hi,
I checked your website but I can see your form.
However I checked your form and see that you have set up a submission limit on your form. This causes the form to be disabled when your submission limit is reached.
I would suggest to remove the submission limit or check the guide here: How to Limit Submissions in a Form.
