Why won't all of my questions show up when I generate a PDF version of the document?

    On both the Patient Satisfaction Survey and the Patient Satisfaction Survey - Spanish some questions don't show up when I generate a PDF.  

     

    Also, I cloned the English version to make the Spanish version and the logo and some of the questions are skewed.  Any idea why?

    Hi,

    Conditionally shown/hidden fields will not be included in the PDF version of the form.  Neither will widgets:

    Several of your fields are conditionally shown and will not be included in the PDF unless the conditions are disabled.

    I was unable to reproduce the issue of the logo being skewed:

    Thank you for the help!!  I will try fixing the conditions. 

     

    For some reason, as I was taking a second screenshot, the logo isn't skewed anymore!  Don't know how...It was just a second ago.  Anyway, thank you for that help also.
