Can embed form in word document?

You cannot embed the form itself on an MS Word Document -- the most you can do is add the link URL to the form, like this http://www.jotform.com/52786133880159  (click on the link)

To be able to embed a form on a web page, it requires the existence of html placeholder or widget at the least.
