    Hello dapoet909,

You may like to take a look the following guide on how to make your form mobile responsive:  http://www.jotform.com/help/311-How-to-make-Forms-Mobile-Responsive

I did check your webpage where you have embedded your form and it seems you have used your form's full source code. Please be noted that for your embedded form to be mobile responsive, we recommend you to embed your form using it iFrame embed code. Please embed your form using its iFrame embed code and we will check the issue you may have. The following guide should help you grab your form's iFrame embed code:  http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

We will wait for your response.

    Thank you!
