The editor is showing the correct form, but preview mode and the embedded form on our website are different from the editor. Have used Google Chrome and Mozilla Firefox and neither are making any difference.
Attached is a screenshot, demonstrating the issue.
I did a check with a cloned copy of you form, and the form preview looked similar to the one in form builder.
Could you kindly specify which portions seem off on your end?
Also it appears the screenshot was not successfully uploaded, you can try re-uploading it again

I did a test on the cloned copy and was able to see the issue you are referring to, the submit button text does not reflect what is on the designer and also the preview dialog is transparent.
I have forwarded this issue to our developers for further investigation.
For now you can click the open in new tab to view the form in a new browser tab, it appears correctly in this view.
