I checked your account/username jbell but I did not find a form on your account. So it must be a form from another user.

The online form won't save/erases the answers so we generated a fill-able .pdf. It is on the fill-able .pdf where the locations to upload documents is not visible.

Unfortunately, you can't incorporate file upload on PDFs that's why it is not supported with Jotform. There's no perfect solution to include the file upload process within the PDF that's why file upload fields are omitted when the PDF is generated. You may however replace it with a text box so users can upload the file somewhere on their server and paste the file URL on the text box.
