    How can I create multiple records for the same family without retyping the information?

    I want to use the form to register our families for our Christmas Giving Tree program. This would be where we accept the request for the size of clothes, shoes, etc for each child in the family. I would like to create a separate record for each child without having to re-type all the family information. What is the best way to register a family where I can enter the family information (address, telephone, email, etc) for the 1st person and then not have to enter the information for the next person/record?

    The way to do that would be to setup the thank you page in such a way to redirect to the same or a similar form with a link that pre-populates the data for them.

    You can of course check if they are going to add another member or if they are finished so that you can show them your thank you page or redirect to the same form with pre-typed entries.

    I am confused about pre-populating fields. It appears that all pre-populated fields are for all entries for that particular form. Every family that I register will have different contact information. After I register the first child in the family and I want to register the 2nd child, using a separate record, is there a way to retain the address, telephone number from the 1st child registration record? Thank you for the clarification and help.

    We would be happy to assist in any step that is not clear, but before that, I have created a short and simple demo to show you what I meant.

    The Parent name and the address will be moved to the new entry if so is selected at the bottom (checkbox option to add another child), or you are taken to thank you page if no more entries are added.

    Thank you for your help. OK, the form works like I want it to. However, I can't confirm if it is creating 2 separate records which I need. I will have about 200 different families that I will be registering (different contact information). Will this still work? 

    You also asked to note the fields that you want to carry over to the next record. It would be all the fields under the Family Contact Information section/page.

    Yes, it is creating several records. You can see them in this grid report: http://www.jotform.com/grid/52865170780056 (I have changed the address and the names since this is a public thread).

    It will work just the same for any number of people that you need to fill out the forms.

    Looking at the form that you have shared with us: http://form.jotformpro.com/form/52855237597974 I presume that you are looking to have all the info passed over except the last page where child details are added?

    Or are you looking to pass only the required fields?

    Based on that, you would need to use this link:

    You should add that link into your thank you page setup (where the users should be redirected to).

    My suggestion is as above - to have a field that asks if a new entry is needed, or if they should be sent to the thank you page.

    OK. I have tried and am not being successful to getting my form to redirect to another registration form. I added the link (you created above) to the form to redirect to a new page to continue with registrations and it does not work for me. Not sure what I am doing wrong.

    If I understood you correctly, you want them to start off at the page where the fields above are present - so that they do not need to go through all of the fields again.

    If that is true, we can append jumpToPage=# in the url.

    If you notice at the top I had added the HTTPS and secure subdomain so that all submissions are being redirected to secure pages.

    Also, while looking into your conditions, I have noticed that there are many entries with deleted fields - please do note that these will cause some issues, so they should be updated (all red entries) to either not point to that specific field, or to be removed completely:

    Thank you for your help! However, when I complete the form, and select "yes" to add another person it does not go to another page.  What am I doing wrong?...I did not change anything you did.  (It just takes me to a page that has a "back" button). Crazy, because I can see that the condition says to redirect.

    I think I was able to remove all the "deleted fields". Thank you for letting me know.

    Just to clarify what I want to do, after I complete all the information on my form and I need to register another participate, I select "yes" to register another participant,  I want to go to another record/form.  

    The following information I would like to have carried forward pre-populated with what was entered on the 1st record

    Number Assigned to the Family

    In addition, these fields would need to be available (NOT pre-filled) to fill out to register the next participant.

    Add another Person  - This would then repeat again if "yes" is selected.

    Now in regards to what you have mentioned, it seems that you are going to the last page (since only back is shown).

    Please do let me know if there was a misunderstanding at one point. I believe that you want to send data from this form http://form.jotformpro.com/form/52855237597974 to the same form (http://form.jotformpro.com/form/52855237597974) once submitted and Yes is selected for another entry.

    I select "yes" to register another participant,  I want to go to another record/form. 

    This is the part that makes me feel that there is another form that it should be redirected to.

    Now as I try to make a test, I see that your submit button is being hidden for you for some reason - so only back is shown.

    I hope again that you will not mind, but I made the changes directly on your form. The reason why the submit button is not shown is because of the following condition:

    - As you can see I have disabled it for you so that I can check your form.

    As I did, I have noticed that the URL did not include the alternative phone number, so I changed the URL to this:

    Just read that post and I see that you've a test form
