My form doesn't show up in Wordpress anymore

    I use Jotform on several wordpress websites with the WP Plugin. My forms don't show up anymore. I tested several websites and several forms and I get the same result each time. I can't see my forms at all.

    When I click on the form, I get an error message: form not found, form disabled. 

    Can you help me with that?

    We apologize for any inconvenience, your account was automatically suspended due to a form flagged as phishing by our Phishing Detector System.

    I have white-listed your form, your account is active again. To avoid this, I would recommend you to change the word "Login", so the system would not score your form as a possible phishing form.
