Looking for way to create anonymous pre/post survey

    Hi,

     

    I'm looking to create anonymous evaluations to be completed before an after an event.  Can you recommend a way to create a unique ID so that I can compare pre/post evaluations without knowing who submitted the survey?  

     

    Here are my forms:

    Currently, both forms ask for the submitter's name but I want to get rid of this field.  I want to be able to compare one submitters responses to each of these forms without knowing who they are.  Can you recommend a tool to do this?

    Thank you for your quick response. I do have a follow up question. 

    We want to anonymize the form so that the user is assigned a Unique ID number that only they know. What if the user forgets to write down their unique ID  number before closing the window? Is there a way for the user to receive an email of their id number without the administrator of the form seeing this information?

    Yes there is a way you can email the ID number, you can create an autoresponder: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

    This autoresponder contains all the data the user has filled out, you can customize it to show only a message and the Unique ID, example:

    Thank you.  I tried this but it seems that the email address still shows up in the View Submissions module, when I am logged in to my account.  

    So it is not completely anonymous.  Is there any way to remove the email address from my view completely?

    Since you are the account owner, you then have full access to all the data of your forms.

    Were you sharing the account to another person? If it is, then that person will have full access to the account as well. 

    It is actually not a good practice to share the account itself to another person.

    You can have another person maintaining and managing your forms using sub account feature instead.

    Using the sub accounts, you can manage what you sub users will be able to access/view in the account.

    OR

    You can also use Encrypted Form so that an access key is needed every time a specific form will be view/open
