What is your JotForm account username or the email address you have used in your account profile? I am unable to find any details with your email address firstname.lastname@example.org. Are you trying to add the link in your form or the link you want to add in the webpage directly?
I will still go ahead and answer your question. Please be noted that you need to add anchor tag to make any text a clickable link. You may like to take a look at the following guide: http://www.w3schools.com/tags/tag_a.asp
