Are you using one of the embed methods explained here ?
If so, which one?
Which page on your site are you trying to embed it on exactly? the home page?

None of the support team (as far as I know) have access to fully working installation of Yahoo Site Builder, but can you try this:
On the Setup & Embed toolbar, click Embed.. then click the Embed icon.
Copy the embed code in the Embed Form text area:
Then in Yahoo Site Builder, from the Insert Menu, select Code Elements , then HTML
Click OK, then save and publish your site.
I am not 100% sure that that will work, but can you please try it and let us know?

I apologize but your account was autosuspended by our system due to phishing that's why your forms aren't appearing after embedded. I have now unsuspended your account so it should be fine now, your forms should now be back online as expected. Please re-embed your form once again and adjust the necessary changes.
Thank you so much for your patience and understanding. Have a great day!

Sorry to say but it is not possible to make login forms with JotForm. We also do not allow them. JotForm doesn't have membership features at this moment. Also, there is no way for us to know if you are using this form for pishing or not. Furthermore, when you attempt to create this type of forms it will be automatically disabled by our system as it is detected as SPAM/Phishing.
I hope this information helps. Thank you so much for asking, have a great day!
