How to allow customers to order multiple items?

    I'm thinking one form for one customer. What if the customer orders multiple items? Would I have to use single form for each item?  I suppose it could use last name and amend a number after it: Jones, Jones1, Jones2....

    I'm not sure if I understand your query correctly. Do you want the form to be submitted only once for each item? You may include a dropdown box with numbers named as quantity so that the customer can select number of items that he wants.
