The fields still exist in the form that people enter into, but that information is not given to me. Can you look into this? I can tell you it is going on currently for me on my SAT registration form.

Is this the correct form in question, http://www.jotformpro.com/form/51732612396961? If yes, would you mind letting us know what fields are missing?
Note that some of your fields aren't required. Meaning, your users will be able to skip these fields. Fields with empty data are hidden in the PDF by default and will show an empty data in the spreadsheet.
Let us know the fields so we can check this further.

The date column (which they do not enter) and the last name column which is a mandatory field.

If you need the date column to be filled, you may consider setting up the fields as required. As for the last name column, users shouldn't be able to skip the required fields. If you have embedded this form, please try using our iframe method (http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code) and let us know if the issue persists after that.
