When you open the form to edit you have to re-select options that are already selected in order for the fields to be hidden. Example on this form Field Employee should hide office comments at the top of the form and Office Comments at the bottom of the form, when opened on edit Field Employee is still selected but Office Comments is not hidden until Field Employee is selected again. We have been using this form for several months now and have not changed any of the conditions recently.

We had an issue earlier with conditions not being checked when forms were loaded. It has since been fixed and you should no longer have to re-select options in order to hide fields.
