I have two questions for you.  I have created a form and selected create PDF and this is the error message I get

This XML file does not appear to have any style information associated with it. The document tree is shown below.

    AccessDenied
    Access Denied

Not sure why however I would like your help if I can.  This will likely be a problem in both the forms created as the second form is a clone of the first.

Also I do not really need a fillable pdf just want the form as it looks in pdf formate as not all the fields show up in the other form I created and not all the labels appear either.

This is the form that will not present all fields and labels https://form.jotform.com/52814112890250

These are the two forms that have the error message above

    Any help would be appreciated

    Thank you for contacting us.

The error is most likely related to the complex form layout. If the fillable PDF is not required, I would like to recommend you to create form PDF file by printing it to the PDF (in Google Chrome go to the Print and then use a Save as PDF option).
