Sharing jotform as Feedback button

Dear Support,

I wish to embed my form in form of feedback button on my website. I have tried this by copying the code for feedback button (Publish>embed>feedback button), and pasting this code in the custom CSS and Javascript. This is not working. I am using wordpress platform and my intention is that this feedback button should be visible on all pages of my website.
