I tried with 2 different cards. When directed to go back and fix, the application is gone and I have to start the process all over again.Page URL:

Are you referring to the form embedded to this page of the site - http://silvereye.org/f16apply/? Are you the owner of the form embedded to the site? I checked the owner of the form and it belongs to jotform user silvereyecenter? The email associated to the account is not the same to the email you use in this thread.
