Embedding form: Too much space between preceding text and the form

    I see nothing in the CSS that is causing this. How can I fix it.

    The issue is somehow related to the layout of your web page. Please try adding the form code after the <ul class="vol"> unordered list.

    You can also try a short Embed code instead of the form source code.
