How can the full form I made plus the signature be sent to me rather than just the signature?

By default, all input fields values including the signature are sent on email. 

I check your form notification and the 3 field tags are intact.

Unfortunately, static texts are not included on email. You can however customize your email notification and paste the text there or whatever you want to add or remove on the notification.
