Is it possible to attach a document to a form?

    You can make your forms able to attach documents by doing the same  as the image below :

    I would like to have a document available on the form for users to access, not giving them access to upload a file.

    Sorry for the confusion. I am assuming that you would like to add a download link in your form. To do this, you would first require to upload your file to the web. You can use dropbox or google drive.

    Once you have uploaded your file, you can use the HTML TEXT field to create the link. To edit the text click the EDIT HTML button.

    Here you can create a link to your file by clicking on the CHAIN icon.
