change user name

Hi,

You cannot change your username in your end, but we can do that for you.

Please provide us 3 possible usernames you would like to change to, just in case the first one is not anymore available.

Please also note that custom URLs of your form will not anymore work because they bear the old username of the account which will not exist anymore when we change your username.
