Worked like a charm,
Now, if I can figure out as to HOW the SENDER can receive a notification that their message has been ' received ' - I'll be a happy camper :)
This is a re-post of a comment on How to Properly Add, Change, and Test a New Email Recipient Address

If you would like to send an email confirmation to the person filling out the form, you would need to set up an autoresponder:
That will send an email to an address entered in the form.
If you have any further questions, let us know and we will be happy to help.

Thank you so much @Mike_G - Support here at JotForm truly ROCKS ✌
You all do an AMAZING job here - and the interface, UI, editor and set-up are simply unrivaled compared to other similar sites which offer the same service.
This is truly the BEST such service site on the web - I love it ❤
