show on one not the outher

I managed to get all working, just one part does not show!!

On firefox.... the header image on the form does not show.

I checked your webpage and there does not seem to be a form embedded.  In the form itself however, the header image showed without issue in both FireFox and Chrome:

Try embedded the form using the iFrame method to see if that helps:
