Can this be used on Android or IPad?

The forms themselves can be filled out from Android or Ipad, however, for creating forms, I would recommend using a desktop/laptop environment.  The forms themselves work quite well on mobile, the form builder tends to be a bit less user friendly on mobile.

I developed a form to gauge the progress of Special Ed students in our classrooms. A big part of that was taking the picture of the evidence to go on the same page as the data collected. Unfortunately, we had planned on using the classroom iPads. They will upload the picture as a file, but not place them on the same page. 

We will try to think of some work around.

I would recommend using Google Chrome for IOS instead of Safari for best results.
