I can not edit my form

We have a form embedded on this page: https://www.unwsp.edu/web/registrar/reenrollment.  When we go to edit this form in JOT, we don't see any of the fields at all.  Only the disclaimer message.  Can you assist?  How do we edit this form?

I have logged in to your account and I could not replicate mentioned issue.

Are you still unable to edit your form inside of Form Builder? You can try to log out and log in again, to see if that will resolve your issue. You can also try with different browser. If you are still unable to edit your form please provide us a screenshot of error message that you are getting. Also please tell us which browser and device are you using.
