The issue was resolved Darren by my colleague pointing out that the form was not being indexed on the intranet (private 'internet'), but that it was a public link that was showing in the results.
Please do note that the only possible way to have your form not indexed is to embed it on your website and place noindex and nofollow meta tags on the same.
That will force the search engines to stop indexing that page and as such the form should not be showing up in the search engines neither.
I do wish to point out that this would mean that your page would not be indexed neither - so that specific value would be removed from the search engines.
