    Restoring to previous version of the form has removed some of the conditions

    I reverted back to an earlier version of this form; however, the conditions in some of my fields are gone. 

    I am not sure if this has anything to do with it, but the last edit I made to my form was a CSS injection to change the colors of the Submit and Print buttons. I don't know if that somehow affected previous versions?

    No, the CSS will not impact the form revisions in such a way. The only issue that might be causing this is if there was something that caused a partial corruption of the data while it was saved in the form revisions.

    Please do let us know what fields are missing conditions (if you remember some) and we could check this out for you to see if there is any way if restoring it.

    Each radio button field had a condition where if "I do not want to contribute/ do not want this coverage" was selected, then the subsequent fields in that section would be hidden. There were also very detailed calculations in the "Premium" fields at the end of each section.

    I also lost some formatting: the alignment in each heading reverted back to "left" after I had "centered" them.

    I can tell you that this was given highest priority by my colleague. I've informed the developer.
