Form disabled

Im creating a website for my university coursework but the forms I've created keep getting disabled, please help. (The form might have been disabled by the phishing detector system)

Please note that forms that would ask for username or email and then a password field are not allowed in Jotform. We consider it as phishing which is against our Terms of Use. 

I have reactivated your account, but I have deleted the password field in your form. Please refrain from adding it again.

Thanks for the reply, but this website is created for my coursework purpose only. It will not be use as a business, If I can't use it can I type password on the hint?

I'm sorry, but we can't make exemptions. I'm referring to the whole field, not just the hint or the label. It's a field that is asking for a password and we don't allow it. Please check our Terms of Use for more details: http://www.jotform.com/terms/.
