I have a form to accept nominations for two different types of awards. The form is setup with conditions and certain questions are asked for awards for individuals and couples and other questions are asked for corporations. I want to turn this form into a fillable pdf. However, when I do, I experience the following issues:
1. The graphics are distorted and stretched throughout the pdf and in between fields.
2. The title of my form and introductory paragraphs are not there.
3. When I complete all fields and hit the submit button, it tells me there are required fields that are not completed.
Aside from those issues, I have an additional question - is there a way to have the pdf generate or change based on the answers? Thus, if someone selects they want to submit a nomination for an individual or couple, I don't want the pdf form to show questions relative to the corporate nomination because that would confuse them. Thus, is there a way around this, or would I need to setup two separate forms (one for individuals and couples and another for corporations) just to have two, separate fillable pdfs?

Apologies for the inconvenience. I cloned your form and converted it to a fillable PDF form.

Please also do read this guide about converting online forms to fillable PDF: https://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator. Please do check the frequently asked questions there. There are things that will not work for the fillable PDF, like the conditional logic and widgets. This means it is not possible to dynamically change or show hidden fields based on a user input when using the fillable PDF.
You can try cloning your form and have two version of it, version 1 would be the online form having all the advance features like conditions and widget, while the other versions are much more simpler, you can remove their conditions and widgets and redesign it to fully work on a fillable PDF version.
