I have one form on a tab for a page that I manage for Community Foundation of Acadiana (CFA).  I am trying to add another form to that particular page.  However, when I try to integrate it on my page, my CFA page does not show up as an option, but two other pages that I manage show up. 

Can I only have one form on Facebook at a time and this is why the CFA page is not showing as an option?  If not, how can I get it to show my CFA page as well?

Unfortunately, Facebook does not allow multiple tabs per application, so JotForm App can only show one form on one tab. However, I believe there's an alternative way to do this using the static iFrame HTML app, you can follow this guide: http://www.jotform.com/help/82-Adding-Form-to-Facebook-Custom-iFrame-App-.
