I tried the condition - if you select "Are you Employed or Self-Employed" if the person selects "Employed" then they would "Please Select" does not give not give any options...Am I doing something wrong?

"Skip to a page or end of the form" Condition is only applicable when you have Page Break Field on your form. Try adding page break tool on your form, it's under Survey Tools that should give you page option inside the condition wizard.

Do I insert a page break at each heading? How do I know what is page 1, 2. or 3?

You can add it anywhere. It's nice if you can add it before each headings, so that every page of your form has it's title.
Every page break on your form has it's equivalent page value on condition wizard.
Your first page break tool is equal to Page 1, then next page break is page 2.
