We ask the customers to choose which kind of customer they are
Kunde type = Type of customer
Privat = Private customer
Virksomhed = Corporate
Partner = partner/reseller
Offentlig = Goverment
If any of above besides private is picked is empty fields shown in the notication email send to management.

Did you mean that the filled fields come empty in the notification email? I just checked your settings and the field names are correct. But if the issue persists, I would suggest re-creating the whole settings.
