I have previously (about a month ago) created a form with conditions which, after embedding on my website, worked then, and still works, exactly as I intended.
I have a new form which I have tried to create with the same process but the system has changed and JotForm has a different "condition" process. I have used this and created a 4-way condition tied as on the previous form, to a radio buttons, but whatever I do, I cannot get the conditions to work on my website after embedding the script on the page.
I have tried with both MS Edge and Firefox, starting form scratch each time, but neither works.
Help!

Hi,
We were experiencing some issues earlier with newly created conditions. This should now be fixed. If you recreate your conditions, they should now work. My apologies. Here is a copy of your form with the conditions recreated:
