Need Dynamic Field to be Four Fields

On Dynamic Fields I need customers to be able to List Quantity, Serial Number, Model Number and Problem Description.  Is there a way to do this with a Dynamic Field?

 Hope you all can easily help me.  I'm thinking your product could be used for all of our locations if this test one works.  I like the look and feel so far!

Hi,

Unfortunately, the dynamic field only allows you to have one input box. You can try to use the Configurable List widget. Configurable List widget allows you to set more than one field with different field types unlike Dynamic field widget.
