How can I fix the form alignment?

    I'm not sure what happened, but my form fields are suddenly very tall and narrow and squished to the left.  Can you tell me how to fix the form alignment and layout to be the full width of the screen, the default settings?

    It looks like there are some modifications made to the form properties on your JotForm. You can revert the changes made to your JotForm by using the Form Revision history. Please refer to the guide below :

    http://www.jotform.com/help/294-How-to-View-Form-Revision-History

    Please see the list of revisions available on the left side and revert to the last working version of your JotForm.
