I am trying to cancel my premium subscription and it keeps saying my username and password is incorrect!

    When I click on "cancel" it takes me to another site to enter my username and password.  When I do, it comes back with a message that my username and password is invalid.  I have done it numerous times.

    We have cancelled your JotForm Premium account. The downgrade will take effect on 2012-02-06 and you will no longer be billed in the future unless you upgrade once again.
