Hi,
If you meant DB, then yes, it is indeed possible to send your form data to your MYSql server. Here is our guide on how to do so:
http://www.jotform.com/help/126-How-to-send-Submissions-to-Your-MySQL-Database-Using-PHP
You will also need to know how to POST submission data to your Thank you page:

If you have any further questions, let us know and we will be happy to help.

Hi,
Unfortunately, that is not possible, since you can not ask for password or users(login credentials) in your forms, if you do it your form will be cataloged as a phishing form and your account will be suspenden or deleted.
You can read our Terms of Use on this link :
Let us know if you have more questions, we will be glad to assist you.
Regards.
