I had create my form - http://www.jotform.com/form/12972410345 and all is very good. But now I have to change e-mail for notification and I can't do it :( It's very bad - but I don't remember: had I registered or not on your service. But your's "forget password" feature not sent e-mail to me on login "fake" (If i had registered - it must be "fake" or "fake101"). Please help me - how to change e-mail in my form?(both e-mails in one domain). Or help me to recovery my login and password to edit form.

That form is under a guest account, meaning you were not logged in when you created that form. I have now manually transferred that form to your account "fake101". Kindly logout and log back in to complete this transfer process. Thank you so much!
