How to create a form with Member vs. Non-Member Pricing with Quantity Discounts

I can't figure out how to create the form in my picture through JotForm. Please help!

I think you will have to create your form with some variants, for example, I would suggest you to add a radio button with to options to your form, member and non-member, then set a condition and show the correct products for each category, to know more about conditions, refer to these guides :

Now, I see in the attached image, you apply a discount depending on the quantity registered, now in order to apply correctly the discounts, I would suggest you to use the Calculation Feature, know more about it on this guide :

And I also assume you will integrate some of the Payment options, then using calculations you can pass the amount to pay to the payment field, these guides will help you to pass values to a payment fields and to integrate a Payment option with your Form :
