What is the best way to integrate a jotform survey with MailChimp?

When I copy the HTML source code of an image survey into MailChimp, it doesn't work. I've read some KB articles indicating that would probably happen, but what should I do instead? I'd like to avoid inserting a link that launches another page. In other words, I'm trying to cut out the middle-man.
